Our Health Information Management (HIM) team:

• Helps you get your health records.
• Manages the UHealth electronic medical record system.
• Makes sure your doctors and nurses can review your records, so you receive safe patient care.

How do I get a copy of my health records?

For a fast and easy way to look at your health records, go online.

• Go to your MyUHealthChart patient portal.
• Click here to go to MyUHealthChart.

Is there an app for my phone to look at my health records?

Yes. You can use your iPhone/iPad or Android phone to look at your health records.

• Go to the App Store or Google Play
• Download the MyChart app
• Use the username and password that you use for the portal.

What if I can’t go online to look at my health records?

To ask for printed copies of your health records fill out and sign a form called:

• Request for Access to Health Information

Please send us the form by fax, e-mail, or mail, or drop it in a locked drop box at the above addresses.

How do I get my health records to UHealth?

1. Call Your UM Doctor: Call your UM doctor for the steps to send your health records to their office.
2. Bring Paper Copies: Bring paper copies of your health records to your appointment.
3. Upload CDs or Images: Contact the Radiology Department to upload CDs or images through MyChart at support.ambrahealth.com.
