Our Health Information Management (HIM) team:
- Helps you get your health records.
- Manages the UHealth electronic medical record system.
- Makes sure your doctors and nurses can review your records, so you receive safe patient care.

How do I get a copy of my health records?
For a fast and easy way to look at your health records, go online.
- Go to your MyUHealthChart patient portal.
- Click here to go to MyUHealthChart.

You can download your records from your patient portal.
Is there an app for my phone to look at my health records?
Yes. You can use your iPhone/iPad or Android phone to look at your health records.
- Go to the App Store or Google Play
- Download the MyChart app
- Use the username and password that you use for the portal.
What if I can’t go online to look at my health records?
To ask for printed copies of your health records fill out and sign a form called:
- Request for Access to Health Information
Please send us the form by fax, e-mail, or mail, or drop it in a locked drop box at the above addresses.

What form do I need?
To ask for printed copies of your health records use:

- Request for Access to Health Information (Spanish)
- Request for Access to Health Information (Creole)
To send your health records to another doctor, organization, or family member use:
- Authorization for 3rd Party Disclosure
- Authorization for 3rd Party Disclosure (Spanish)
- Authorization for 3rd Party Disclosure (Creole)
How do I get my health records to UHealth?
- Call Your UM Doctor: Call your UM doctor for the steps to send your health records to their office.
- Bring Paper Copies: Bring paper copies of your health records to your appointment.
- Upload CDs or Images: Contact the Radiology Department to upload CDs or images through MyChart at support.ambrahealth.com.
